All Stakeholder Meeting: August 26, 2026 11am – 1pm ET in Jasper, Indiana

EFI will feature a live example of a direct-to-employer arrangement on August 26 in Jasper, Indiana. The event will highlight the direct agreement between EFI member Patoka Valley Health Cooperative and Jasper Holdings, in partnership with Deaconess Health System. Panelists will include:

New Law-to-Action (LTA) Brief: Anticompetitive Contract Clauses

EFI has released its latest Law-to-Action, focused on anticompetitive contract clauses and the new Indiana laws that give employers greater plan oversight and benefit design authority.

It outlines how employers can identify contract provisions that limit steering, tiering, carve-outs, data access, audits, and other purchasing strategies that can interfere with effective oversight of TPAs, PBMs, and healthcare vendors. It also highlights practical steps employers can take to review contracts, negotiating renewals, and when evaluating vendor arrangements. View the LTA library here.

Sage Transparency 2.0: See and Compare Hospital Pricing in Your Market

As transparency laws take hold, employers are being asked a basic question: does what you pay for healthcare reflect fair value? That kind of analysis requires a tool built for the job.

Sage Transparency 2.0 brings together hospital pricing, quality, and financial data in a single searchable platform covering more than 4,000 hospitals and 2,000 ambulatory surgery centers nationwide, with peer benchmarking and Medicare comparisons built in.

A new video shows how the platform works and how employers can start using it. If you haven’t explored Sage Transparency yet, this is a good place to start.

All Stakeholder Meeting Recap: Advanced Primary Care with Linda Brady

On May 20th, EFI was hosted by Fort Wayne Community School Corporation for our first regional All Stakeholder Meeting. At the event, Linda Brady shared Boeing’s journey into advanced primary care, a strategy the manufacturer began over a decade ago that has grown into several near-site clinics across multiple markets. Key elements of Boeing’s approach include:

  • Ensuring primary care is comprehensive and includes informed referrals to the highest-quality, best-value specialists
  • Moving away from fee-for-service funding so providers have adequate time with patients
  • Prioritizing timely access to care as a core design principle
  • Boeing’s approach has led to improved outcomes and patient experience, greater access to primary care, and cost savings for both the company and employees.

Three Indiana employers also shared their own primary care models, which varied from near-site clinics to dedicated onsite primary care. Like Boeing, Indiana employers are also seeing better access to care, improved outcomes, and cost savings. One employer discussed adding GLP-1 management as a new offering through their onsite clinic.

The consistent message, given the positive results they’ve seen, is that employers are looking to grow their direct primary care models further.

Indiana Takes on Non Profit Hospitals

Indiana is taking on powerful nonprofit hospitals by incentivizing them to lower their prices or face the loss of their nonprofit status by 2029. Governor Mike Braun has been direct about the rationale: “Government has to intervene, because healthcare is run like an unregulated utility.”

For EFI member Doug Bawel, Chairman of Jasper Holdings, the law’s requirement that nonprofit hospitals offer direct-to-employer arrangements is already reshaping negotiations. Bawel said he expects the state’s price controls on direct deals to “significantly strengthen his negotiating leverage with hospitals.”

Creating the law is just the beginning. Leveraging it is the work ahead, and the August 26 Jasper event will focus on exactly that.

When Coverage Erodes, Employers Pay More

Indiana hospitals report a 17% surge in emergency department visits, driven by a wave of uninsured patients after ACA subsidies expired and Medicaid rolls were cut. When uncompensated care rises, hospitals often use it to argue for higher commercial prices. EFI’s Jonathan Levin pushed back in the Indiana Capital Chronicle:

“Indiana employers already pay some of the highest hospital prices in the country… the available evidence has not supported the conclusion that higher employer-sponsored insurance prices are necessary to offset increases in uncompensated care.”

Is ERISA Up for the Job? New Georgetown Report

Georgetown’s Center on Health Insurance Reforms just published a close look at whether ERISA, the federal law governing most employer health plans, is equipped to control costs. The short answer: it has real tools, but Congress could do more.

The report examines how ERISA’s fiduciary rules, transparency requirements, and Department of Labor enforcement can be used and strengthened to rein in PBMs, TPAs, and other service providers. Worth a read for anyone navigating plan governance or vendor negotiations.

EFI Welcomes Jonathan Levin, MPH, PhD

Jonathan Levin

We’re excited to welcome Jon Levin to EFI as Vice President of Healthcare Economics and Policy Research.

Jon joins EFI from RAND, where his work focused on the drivers of healthcare spending, including hospital prices, prescription drug costs, and consolidation, as well as the role of payment and delivery system design in improving the value employers and patients receive.

He’ll be a great addition to the team as EFI continues to expand its research and policy work on healthcare affordability.

New Resource: Law to Action – Data Access and Audit Rights

A wave of new Indiana laws, combined with recent federal legislation, gives plan sponsors explicit legal authority to access complete, unredacted claims data and to better oversee their plan expenses. TPAs and PBMs are required to provide access to payment data that employers have rarely had access to before Indiana’s groundbreaking laws.

EFI’s latest Law-to-Action guide covers what the new laws require of your vendors and four concrete steps to act on your rights now.

New Report: Setting the Record Straight on Hospital Prices

The National Alliance of Healthcare Purchaser Coalitions recently released a new report examining claims often used to justify rising hospital prices.

The report highlights how hospital consolidation, opaque pricing practices, and limited competition continue to drive healthcare costs higher for employers and workers. Employer-sponsored coverage now averages nearly $27,000 per year for family coverage, underscoring the financial pressure facing businesses and employees. 

The report also outlines steps employers and coalitions are taking to bring greater transparency and accountability to hospital pricing.

PT6 (RAND 6.0): Help Shape the Next Study

Enrollment is now open for RAND 6.0, the latest national study examining commercial hospital prices.

The RAND hospital price studies have been instrumental in documenting price variation across the country and informing transparency reforms. Benchmarking hospital prices against Medicare is a basic reference point for responsible plan oversight. Participating in RAND helps strengthen the data employers need to evaluate their contracts and supports the transparency behind Sage.

EFI encourages all employers to consider participating in this round of the study.

INDIANAPOLIS, IN – The Employers’ Forum of Indiana (EFI) is pleased to announce that Jonathan S. Levin, PhD, MPH, has joined the organization as Vice President of Healthcare Economics and Policy Research. Levin has more than a decade of experience studying what drives healthcare costs, how payment systems work, and where they break down. He brings that work to an organization with deep policy roots and a track record of turning research into real change, in Indiana and increasingly across the country.

Levin most recently served as a Policy Researcher at RAND, leading teams of data scientists, economists, clinicians, and statisticians and directing nearly $1 million in competitive grants as Principal Investigator. His research has focused on payment reform, provider consolidation, prescription drug costs, and the relationship between hospital prices and the quality-of-care patients actually receive. He has published more than 25 peer-reviewed articles in journals including Health Affairs, JAMA Network Open, Health Services Research, and Health Economics. His work has informed policy decisions at the federal, state, and local levels, with findings presented to federal health agencies, state Medicaid programs, and county governments across the country.

Levin’s research experience aligns closely with EFI’s focus on advancing transparency, value, and affordability in healthcare. RAND powers Sage Transparency 2.0, EFI’s free, open-access platform that gives employers, policymakers, and journalists the ability to compare hospital prices and quality in one place. It is the kind of tool Levin’s research was built to inform.

“Jonathan has spent years studying exactly the problems our members and state are working to solve,” said Randa Deaton, M.A., Chief Executive Officer of the Employers’ Forum of Indiana. “He understands the key cost drivers of employer- sponsored insurance, and how to turn complicated data into evidence-based insights that will guide EFI’s priorities. We are thrilled to have him leading our transparency and research initiatives.”

Levin holds a PhD in Health Policy and Management from Johns Hopkins University, an MPH from Harvard University, and a BA from Oberlin College. As Vice President of Healthcare Economics and Policy Research, Levin will oversee EFI’s research and data analytics work, support the organization’s state and federal policy efforts, and serve as a subject matter expert for EFI members, legislators, and strategic partners.

Indiana Hospital Association (IHA) President Scott Tittle’s recent op-ed attempts to create confusion around data, distorts the Employers’ Forum of Indiana’s (EFI) analysis, and relies on the IHA’s strategy of cherry-picking rural hospital examples while shielding the major hospital systems that have been the focus of policies at the statehouse.

The IHA misrepresents EFI’s position by asserting that our analysis accuses hospitals of overstating their financial challenges, but EFI made no such claim. Our analysis found that Indiana’s hospital market is bifurcated, with some hospitals clearly struggling and others extremely profitable, resulting in significant variation in financial performance across the state. That finding aligns with national research, including the RAND 5.1 hospital price transparency study. RAND 5.1 further shows that Indiana has the ninth highest hospital costs in the nation and that Hoosiers with employer-sponsored insurance pay approximately 300% of what Medicare would pay for the same services.

Recognizing that variation in financial performance is not skepticism of hospital distress; rather, it is necessary to protect access to care where it is most at risk while appropriately accounting for the financial capacity of the state’s most profitable hospitals.

The IHA-commissioned study relied on an eight-month snapshot of 53 undisclosed hospitals. EFI’s analysis, by contrast, included a full year of data from the state’s 114 general acute care hospitals using Medicare Cost Reports prepared, signed, and submitted to Medicare by hospital executives.

EFI accessed the data through the Hospital Cost Tool, a publicly available data source managed by the National Academy for State Health Policy (NASHP), which sources the data directly from the CMS-hosted database called HCRIS. Moreover, EFI published both its analysis and the underlying data sources so the findings could be reviewed, evaluated, and replicated.

EFI used this data in its analysis for several reasons:

  • It contains 14 years of financial information covering over 4,000 individual acute care hospitals in all 50 states in one database.
  • The information is in a common format with consistent definitions.
  • It covers hospital operations, which constitute the core of our health systems.
  • Audited financial statements are generally not available for all hospitals at the individual hospital level, report formats vary among hospitals, and these reports are not available in a searchable database.

Medicare Cost Reports do not include the revenue and expenses of hospital subsidiaries, such as affiliated medical groups or real estate ventures, because they are designed to measure the financial performance of hospital operations. That focus is intentional. For purposes of informing public policy, the relevant question is whether hospitals themselves have the overall financial capacity to sustain operations.

Over time, hospital systems have expanded into a wide range of non-hospital enterprises, and hospitals may subsidize some of those activities, including affiliated medical groups. As a result, the financial information reported in Medicare Cost Reports and in system-level audited financial statements may differ. Those differences do not indicate flawed data but reflect different scopes and different questions being asked.

The subsidy of physician groups is itself a recognized issue. As documented in the RAND hospital price transparency studies, hospitals in Indiana are paid consistently above the national average, while physician groups are paid below the national average, contributing to consolidation and higher overall prices. That dynamic underscores, rather than undermines, the importance of separating hospital operations from broader system finances when assessing financial capacity and shaping targeted public policy responses.

EFI shares IHA’s concerns regarding the financial distress of several of Indiana’s hospitals. Moving forward, we invite the IHA to consider innovative strategies for hospitals and employers to work directly together to support the vital role that businesses and hospitals play in creating healthier communities.

EFI members are invited for a very special All Stakeholder meeting on December 3rd for a private screening of The Tenderness Tour film. It’s a beautiful story of award-winning activist, Richard Propes, a cancer-survivor, double amputee with spina bifida, traveling through Indiana on a ‘Tenderness Tour,’ to raise awareness about the issues of the medical debt crisis in Indiana and the United States. Richard Propes traveled 150 miles across Indiana in 2024 for the Tenderness Tour raising over $100,000 to eliminate debt for Indiana families.

EFI member Jenni Nolan, who founded Clear Healthcare Advocacy, is featured in the film and will be joined by Andie Redwine, the film’s director.

This meeting will feature Antonio Ciaccia, President of 3 Axis Advisors and a nationally recognized expert in healthcare drug pricing and policy. Antonio will provide an overview of the current industry players, drug pricing trends, and opportunities to address drug affordability. 

Karen Handorf, J.D., a Full Professor of the Practice at Center on Health Insurance Reforms (CHIR) and an ERISA attorney, will provide an update on the latest employer lawsuits and discuss strategies for mitigating risk through fiduciary oversight and contracting.

To round out the meeting, Jessica Brooks-Woods, CEO of the National Association of Benefits and Insurance Professionals, will join me on stage to discuss how employers can work with their brokers and consultants to drive drug affordability and accountability.

The EFI Team is thrilled to welcome Randa Deaton as our new President and CEO. Take a few moments to get to know her with the below quick Q/A!

What drew you to the Forum and what project or initiative are you most excited to work on? 

I could not be more excited and honored to join EFI; I consider it one of the most influential and impactful state employer coalitions in the country. After working for many years in this space in Kentucky and at a national level, I am most excited to be working and collaborating with employers in my home state of Indiana to drive improved health care affordability and quality on behalf of Hoosier workers and families.

What inspired you to pursue a career in this industry, and how has your journey shaped your perspective on healthcare?

My passion for improving health care comes from both my personal and professional experiences over the last two decades. As part of my role in benefit strategy at Ford Motor Company, I was charged with engaging and collaborating with peer businesses in Greater Louisville to improve healthcare quality and affordability; here I learned the power of collective impact to drive improved transparency, accountability, and health outcomes. I also spent over a decade as a caregiver to a family member with a serious illness, and here I learned what exceptional and accessible care looks and feels like and its extraordinary impact on individuals and families. I believe we should all be working together to ensure access to exceptional care that optimizes outcomes.

What are some of your favorite activities or hobbies outside of work?

My husband and I love to travel, camp, watch sports, and hang out with our adult sons. We spend half the year weekend camping (or glamping) at Indiana State parks with friends and family hiking, biking, and sitting around the fire. The other half of the year is spent at IU football and basketball games cheering on the Hoosiers – a family tradition from both of our families.

INDIANAPOLIS — The Employers’ Forum of Indiana (EFI) is pleased to announce the appointment of Randa Deaton, M.A., as its new Chief Executive Officer effective July 21, 2025.

Before joining EFI, Deaton served as Vice President of Purchaser Engagement for the Purchaser Business Group on Health (PBGH), a national nonprofit organization representing 40 of the largest public and private purchasers who collectively spend $350 billion on healthcare annually for 21 million American workers and their families. In this role, she led the member value team in engaging purchasers with high-impact solutions to enhance healthcare quality, reduce costs, and improve population health.

Prior to her time at PBGH, Deaton served as President and CEO of the Kentuckiana Health Collaborative, bringing over 17 years of experience working directly with healthcare purchasers to drive improvements in the healthcare system in Kentucky and Southern Indiana. She also led the Corporate Community Health Initiative as part of the benefit strategy team for Ford Motor Company from 2004 to 2020. In her previous roles, she led the development of PBM and maternity contracting standards for employers, as well as Kentucky’s first multi-payer primary care public reporting, utilizing the statewide quality measures developed under her leadership.

Deaton has served in a variety of advisory roles as Vice Chair of the National Alliance of Healthcare Purchaser Coalitions, Employer Taskforce Chair for the Biller Family Foundation, Advisory Board Member of the University of Louisville’s School of Public Health and Information Sciences Advisory Board, Advisory Member to the Greater Louisville Medical Society’s Quality Improvement and Patient Safety Committee, and Taskforce Member of the National Quality Forum’s (NQF) Driving Value through the Next Generation of Quality.

“I’m thrilled to return to my home state at a time when employers have a vital role in transforming healthcare,” said Deaton. “EFI has an incredibly strong foundation, and I look forward to connecting with members and elevating the voices of Indiana employers in health policy and practice.”

Deaton holds a master’s degree in industrial/organizational psychology from Middle Tennessee State University and a bachelor’s degree in psychology from Indiana State University. In her personal time, she enjoys camping, traveling, and cheering on Indiana sports teams with her family.

Our free, online tool Sage Transparency 2.0 has once again been updated! As of April 8, the following data were updated:

NASHP Hospital Cost Tool:

  • Financial data updated to reflect 2023 numbers

CMS Hospital Star Ratings:

  • Ratings were updated to reflect the CMS release on February 19, 2025

Quantros Hospital Quality Reliability Scores:

  • Scores updated to reflect new data

January Advisors:

  • Legislator mapping data updated to reflect 2024 elections

2025 Indiana Legislative Healthcare Summary

The 2025 Indiana Legislative session was one for the books and focused heavily on healthcare. The Forum helped to pass meaningful legislation aimed at addressing healthcare prices, consolidation, and accountability. With her extensive background in healthcare benefits and employee wellness, Candace brings a wealth of knowledge and strategic insight that will greatly benefit our mission and initiatives.

Candace currently serves as the Associate Vice President of Benefits & Payroll at Purdue University, where she leads the design and administration of benefits and payroll programs for faculty and staff.
